Under the direction of the Registered or Licensed Practical Nurse, is responsible for observing the patient and reporting observations to the RN/LPN to protect the patient from injury or harm to themselves or others and to prevent elopement.

Continuously monitor patients to ensure their safety and promptly identify behaviors that may lead to self-harm, harm to others, or elopement.

Observe and document patient behaviors, conditions, and changes and accurately communicate observations to the Registered Nurse (RN) or Licensed Practical Nurse (LPN).

Provide one-to-one patient supervision as assigned to maintain a safe and secure environment.

Assist in preventing patient injury and elopement by remaining alert, following established safety protocols, and reporting potential risks immediately.

Collaborate with nursing staff to support patient care plans and ensure timely intervention when safety concerns arise.

Performs related duties as required.

Required:Education: High school diploma or GED.

Preferred:Education: Enrollment in a professional nursing school with completion of fundamentals of nursing (lecture and clinical), and currently in first year of clinical rotation.
