Responsibilities

• Perform comprehensive in-home assessments and provide personalized, holistic care.

• Assist with Activities of Daily Living (ADLs) as needed with dignity and respect.
• Coordinate care with patients, plan of care, and collaborating physicians.
• Monitor patient symptoms and report changes promptly to care team.
• Administer medications and treatments as prescribed by the physician.
• Maintain accurate patient documentation in the EMR system.

Responsibilities

• Be a Pillar of Support:You'll conduct thorough assessments, ensuring each patient receives personalized, comprehensive care.

Qualifications

• A valid and active RN License in the state of practice.
• A willingness to travel, bringing care directly to patients' homes.
• Graduation from an accredited school of nursing.
• Current BLSCertification.
• Ability to pass a state & federal background check, ensuring trust and safety.
• Malpractice Insurance is required.
• Strong problem-solving and critical thinking skills, coupled with a warm, empathetic approach.

Pay Range

USD $38.00 - USD $40.00 /Hr.
