Qualifications

• Active RN license in the state of practice.
• Graduation from an accredited school of nursing.
• Current BLS certification.
• Ability to pass state and federal background checks.
• Malpractice insurance is required.

Responsibilities

• Conduct thorough patient assessments and ensure personalized, comprehensive care.
• Educate patients and families on health maintenance and disease prevention.
• Provide assistance with Activities of Daily Living (ADLs) with dignity and respect.
• Coordinate schedules with patients according to their care plan and preferences.
• Monitor patient symptoms and promptly report changes to the care team.
• Administer medications and treatments as prescribed by the physician.
• Communicate with Case Managers and physicians for a unified care approach.
• Document patient care accurately in the EMR system.

Responsibilities

Pay Range
