Position: Registered Nurse - RN - Hospice - PRN - Flexible Schedule

- This position will serve as backup for On Call

- Must be available for after hours - 5p-8a.

Responsibilities of a Hospice RN/ RN Case Manager:
  1. Report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.
  2. Act as the primary liaison between the physician, patient/family, and hospice team.
  3. Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload while fostering trusting and compassionate relationships.
  4. Empower patient’s caregivers by educating them on care for their loved one.
  5. Manage and collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
  6. Evaluate patient and family needs and assist in the development and implementation of the care plan.
  7. Maintain all clinical records for assigned patients.
  8. Participate in quality assessment and performance improvement programs.
About You - Hospice Registered Nurse Requirements:
  • Compassionate, kind, and empathetic to patients and families.
  • Comfortable providing care wherever the patient calls “home”.
  • Demonstrate flexibility and composure during crises.
  • RN license in the requested state or eligibility to obtain one.
  • At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital staff RN, Med Surg, PACU, ER, critical care, or similar settings is a plus.
  • Valid driver’s license and auto insurance.
  • Current CPR certification.
