Job summary

VitalCaring in Alabama is seeking a Hospice Registered Nurse (RN) to lead compassionate, patient-centered end-of-life care in homes and community settings. You will coordinate plans of care, manage symptoms, and provide education and reassurance to patients and families with empathy and clinical excellence.

This field-based role offers high autonomy with team support, a focus on comfort and quality of life, and the chance to make a meaningful difference during life's most important moments.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Responsibilities

• Lead Patient-Centered Hospice Care - conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with focus on pain and symptom management
• Serve as the Care Coordinator - case management across IDT
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort
• Provide education and emotional support to patients and families
• Help families navigate complex decisions with empathy and clarity
• Maintain accurate, timely documentation and participate in care planning

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

What You’ll Do

Lead Patient-Centered Hospice Care

Drive Quality & Compliance

• Maintain accurate, timely documentation
• Participate in IDT meetings and care planning
• Support quality and compliance standards

What You Bring

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
