Job summary

OhioHealth at Home in partnership with Compassus is seeking a Hospice Registered Nurse (RN) Case Manager to join a caring team in Ohio. This role involves weekend shifts (Saturday and Sunday, 12 hour shifts) with a focus on patient and family education, symptom management, and collaborative care planning.

Responsibilities include building trusting relationships with patients and families, empowering caregivers, and coordinating with the care team to provide comfort, peace, and dignity.

Qualifications

• RN license in the state you work
• Two years of experience as a registered nurse, hospice experience preferred
• Current driver’s license
• Current CPR and BCLS certification
