Job summary

Trellis Supportive Care in Salisbury, NC is seeking a Registered Nurse (RN) Hospice Case Manager – Homecare to deliver compassionate end-of-life care in patients' homes and facilities.

You will develop and implement plans of care, coordinate with an interdisciplinary team, and reassess as needs evolve, ensuring comfort and dignity for patients and families. A sign-on bonus is available.

Location: Serving patients and families in Rowan County and surrounding counties
Office Location: Trellis Supportive Care – Salisbury, NC

Schedule

• Full-Time (40 hours per week)
• Five 8-hour weekday shifts
• Occasional back-up on-call rotation
