Job summary

St. Elizabeth Healthcare is hiring a Hospice Care Coordinator for the Edgewood, KY hospice unit. This role is 1st shift and PRN, with less than 20 hours weekly.

Responsibilities include establishing community connections, coordinating transfers for continuity of care, and educating the community about the Hospice Philosophy of Care.

Qualifications

• Graduate of an accredited nursing program and licensed in the state of practice.
• Compact (multi-state) license eligibility preferred.
• 2–3 years RN experience in a hospital or care facility setting.
• FLSA Status: Non-Exempt.

• Facilitate client transfer between the facilities and the hospice home care and inpatient unit to ensure continuity of care.

Qualifications

• Education, Credentials, Licenses: Graduate of an accredited school of nursing; licensed to practice in the state where work is being performed. If eligible, must have a Compact (multi-state) License in their primary state of residence. Meets contact hour requirements for licensure, including all state-required courses. Valid Driver’s license.
• Specialized Knowledge: Demonstrates advanced assessment skills appropriate for home care environment, ability to function independently; knowledge of community resources.
• Kind and Length of Experience: 2-3 years of RN experience in a hospital or care facility setting.
• FLSA Status: Non-Exempt Right Career.
