Job summary

Klarus Hospice is seeking compassionate and dedicated Registered Nurses (RN) to join our team in San Antonio, TX and surrounding areas. We provide hospice care to community-based patients and support geriatric wellbeing with comfort and dignity.

Join Klarus Hospice under Jet Health and contribute to patient-centered home care, documentation, and coordinated services using modern tools and travel within designated territories.

Qualifications

• Current State or Compact RN License.
• Minimum of one year clinical experience preferred.
• Oasis knowledge and/or end-of-life care experience preferred.
• Demonstrate knowledge and skillset in current nursing practice.
• Possess a valid state driver's license and automobile liability insurance.
• Knowledge of HCHB EMR is desired.

Responsibilities

• Follow designated care plan in accordance with patient needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including medication administration, wound care, treatments, and procedures.
• Monitor client conditions and report changes to the Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors' orders.
• Perform client assessments.
• Provide case management and coordination of care.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
