Job summary

Home Health Care, Inc. is seeking an experienced RN Visit Nurse to provide skilled nursing care in patients' homes across Minnesota.

You’ll work independently with an established clinical team behind you, visiting patients in their homes to assess needs, deliver care, educate families, and document findings. Responsibilities

• Provide skilled nursing care directly in patients' homes.

What Your Visits Could Include

As an RN Visit Nurse, you'll travel throughout your assigned area providing skilled nursing care directly in patients' homes.

Your visits may include:

• Wound care and wound assessment
• Medication reconciliation and management
• Injections and other skilled nursing procedures
• Monitoring chronic and acute medical conditions
• Patient and caregiver education
• Identifying changes in condition
• Communicating concerns and updates with physicians
• Coordinating with PT, OT, SLP, social work, and other members of the care team
• Helping patients understand how to manage their health safely at home
• Completing timely documentation in Homecare Homebase (HCHB)
