Duke HomeCare & Hospice in Durham, NC is seeking an experienced Registered Nurse (RN) to provide high-quality home health care. In this on-site role, you will assess patients, formulate care plans, administer treatments, and coordinate with interdisciplinary teams to improve outcomes while ensuring compliance with NC regulations.

• Associate Degree or Diploma in Nursing is required; BSN encouraged after two years of service.
• Current RN license in North Carolina or compact state.
• CPR certification (American Heart Association).
• Valid driver’s license and current automobile insurance coverage.
• 1–2 years of med-surgical, critical care, or home health experience preferred.
• Knowledge of Medicare/Medicaid regulations, nursing scope, and infection control standards.
• Strong assessment, critical thinking, communication, and documentation skills; ability to work independently.

Responsabilidades

• Deliver and document professional nursing care in home settings per nursing process and NC regulations and agency standards.
• Conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams.
• Educate patients and families on disease processes, medications, and care plans to support outcomes and reduce readmissions.
• Coordinate care including case management, discharge planning, and OASIS assessments tied to reimbursement and quality outcomes.
• Ensure compliance with regulatory, safety, and quality standards while maintaining accurate and timely documentation.

Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

How you will make an impact
