Job summary

Five Star Home Health Care LLC seeks a Registered Nurse (RN) for PRN home health visits in Chantilly, VA. The RN will perform 1-on-1 care in patients’ homes, documenting care, and coordinating with families and supervisors. This is an as-needed, in-person role with Monday-Friday availability in a home health setting.

Ideal candidates hold an active Virginia RN license and have at least one year of acute care experience. Home health experience and HCHB EMR familiarity are preferred.

Qualifications

• Active RN license in Virginia is required.
• One year nursing experience in acute care is required.
• One year home health experience is preferred.
• Experience with HCHB EMR is preferred.
• Bachelor’s or Associate degree in nursing required/preferred

Responsibilities

• Provide 1-on-1 nursing care in patients’ homes.
• Render PRN nursing services as needed.
• Collaborate with patients and families to develop care plans.

Qualifications

• Graduated with an associate or bachelor's degree in nursing
• One (1) year nursing experience in acute care is required
• One (1) year home health experience is preferred
• Experience with HCHB EMR is preferred
• Active RN license in Virginia is required
