Job summary

ABL Health Care LLC in Pleasanton is seeking a compassionate Full-Time Registered Nurse (RN) for our Home Health team. You will evaluate patients, create and implement care plans, and coordinate with the clinical team to deliver high-quality home-based care.

The ideal candidate is licensed in California, experienced in home health or hospice, and able to travel within the service area. We offer competitive compensation and a comprehensive benefits package.

Qualifications

• Current Registered Nurse license to practice in California (Required).
• Experience in Home Health or Hospice care.
• Current CA Driver's License.

Responsibilities

• Provide nursing care within the scope of practice, following the patient’s individualized care plan in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report changes to Physicians, Case Manager, and Director of Nursing as needed.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.

Position Overview:

We are seeking an experienced, self-driven Full-Time Registered Nurse (RN) to join our dedicated Home Health team and provide exceptional care throughout the region. As a Registered Nurse (RN), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.
