Job summary

ABL Health Care LLC in Daly City is seeking a dedicated Registered Nurse (RN) for Full-Time Home Health to join our patient-centered team. The RN will evaluate patients, create care plans, and coordinate care with the interdisciplinary team, while traveling to patient homes across the region.

The ideal candidate holds a California RN license, has Home Health or Hospice experience, and is committed to delivering compassionate, high-quality care with strong documentation and communication skills.

Position Overview

We are seeking an experienced, self-driven Full-Time Registered Nurse (RN) to join our dedicated Home Health team and provide exceptional care throughout the region. As a Registered Nurse (RN), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Requirements

• Current Registered Nurse (RN) License to practice in California - Required
• Experience in Home Health or Hospice care
• Current CA Driver's License
