Job summary

ABL Health Care in Mountain View is seeking a dedicated Registered Nurse (RN) for full-time Home Health. You will assess patient needs, develop and implement care plans, and collaborate with the interdisciplinary team to deliver high-quality home-based care.

Required: California RN license, experience in home health or hospice, valid CA driver's license, and ability to travel. You will work with a supportive team focused on comfort, dignity, and patient-family engagement.

Responsibilities

• Provide nursing care within the scope of practice, following the patient's individualized care plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, and Director of Nursing.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.

Position Overview

We are seeking an experienced, self-driven Full-Time Registered Nurse (RN) to join our dedicated Home Health team and provide exceptional care throughout the region. As a Registered Nurse (RN), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You\'ll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Responsibilities

• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.


• Supervise Licensed Vocational Nurses (LVNs) when assigned.


• Comply with all company policies and procedures to ensure high standards of care

Requirements

• Current Registered Nurse (RN) License to practice in California - Required
