Registered Nurse (RN) - Home Health Case Manager - Far Southeast Suburbs

Position Overview

This position is located in the Far Southeast Suburbs, focusing on areas including Lansing, Calumet City, Riverdale, Dixmoor, Dolton, South Holland, Burnham, Glenwood, and Thornton. The role involves working days, with requirements to work one summer and one winter holiday, and participate in an on-call schedule. It is eligible for a sign-on bonus of up to $10,000, contingent upon employment at payout.

Major Responsibilities
  1. Nurse/Patient Relationship: Develops and maintains therapeutic relationships, assesses patient and family readiness, involves them in care planning, and uses a holistic approach.
  2. Assuring/Improving Quality Care: Ensures delivery of quality, cost-effective care, mobilizes resources, adheres to standards, and participates in quality initiatives.
  3. Patient/Staff Education: Meets learning needs, utilizes resources, and promotes educational growth for staff.
  4. Nursing Process: Utilizes nursing process for problem-solving, recognizes clinical changes, makes clinical decisions, prioritizes care, and coordinates referrals and discharge planning.
  5. Leadership/Collaboration/Professional Development: Participates in team activities, demonstrates leadership, maintains current knowledge, and earns required certifications.
Licensure and Education Requirements
  • Valid Illinois RN license
  • Graduate from an accredited nursing program
  • Minimum of 10 continuing education credits annually
  • At least one year of medical-surgical experience

Physical Requirements and Working Conditions

  • Ability to lift up to 35 pounds without assistance, with patient handling equipment used for heavier lifts
