Registered Nurse (RN) - Home Health Case Manager Chicago Southeast

Join us as a Registered Nurse (RN) - Home Health Case Manager at Advocate Health in Chicago Southeast.

Location: Southeast City Communities

Hours: Days with rotation to weekends and holidays

This position offers a sign-on bonus of up to $10,000, payable when currently employed in the role at payout time. Internal candidates are not eligible.

Major Responsibilities
  1. Nurse/Patient Relationship: Develops and maintains therapeutic relationships, demonstrating adaptive communication, providing individualized care, assessing readiness, and involving patients and families in planning.
  2. Assuring/Improving Quality Care: Facilitates quality, cost-effective care, mobilizes resources, adheres to standards, participates in quality initiatives, and ensures safety and regulatory compliance.
  3. Patient/Staff Education: Meets learning needs, utilizes resources, and promotes educational growth among staff.
  4. Nursing Process: Applies nursing problem-solving, recognizes clinical changes, prioritizes care, coordinates referrals, and maintains confidentiality and ethical standards.
  5. Leadership/Collaboration/Professional Development: Participates in team activities, demonstrates communication skills, seeks certifications or degrees, and maintains ongoing education.
Licensure and Education
  • Registered Nurse (RN) license in Illinois
  • Graduate from an accredited nursing school
  • Minimum of 10 continuing education credits annually
  • One year of medical-surgical experience

Physical Requirements and Working Conditions

  • Work environment includes exposure to hazards and infectious materials
  • Ability to lift up to 35 pounds without assistance, with patient handling equipment used for heavier lifts
