Responsibilities: As a Home Health RN, you will:

• Provide home-based care to patients with various health conditions, often post-discharge.
• Work with physicians and oversee multidisciplinary teams.
• Assess patients, document progress, and report to physicians.
• Educate patients and families on care plans, medications, and safety.
• Coordinate discharge and ensure care goals are met.

Required Skills:

• Graduate of an accredited RN program
• Active, unrestricted RN license in South Carolina or multi-state license
• Minimum of one year of nursing experience, preferably in home health, Med/Surg, or Critical Care
