Job Description:

BAYADA Home Health Care is seeking a Registered Nurse (RN) Home Health for a nursing job in Fairfax, Virginia. This office services our adult and geriatric clients in assisted living and independent living communities on a per visit basis throughout Northern Virginia. This is a Full-Time position.

Responsibilities:

1. Follow a designated care plan in accordance with patient's needs.
2. Make home visits to clients in designated geographic territories.
3. Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
4. Monitor clients' conditions; report changes to Clinical or Client Services Manager.
5. Follow up with, execute, and properly document doctors' orders.
6. Perform client assessments as necessary.
7. Case management and coordination.
8. Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications:

1. Graduation from an accredited and approved nursing program.
2. A current license as a Registered Nurse in Virginia.
