Job Summary

The RN administers skilled nursing care in patients' homes, following physician's orders and care plans, under the supervision of the Director of Clinical Services/Nursing Supervisor.

Responsibilities

• Perform services according to the care plan.
• Conduct initial and ongoing patient evaluations.
• Develop and revise care plans.
• Provide specialized nursing services.
• Document patient visits and care conferences.
• Coordinate services and communicate patient needs.
• Educate patients and families.
• Participate in in-service programs, peer review, and performance improvement.
• Complete assessments, orders, and discharge summaries timely and accurately.

Requirements

• Graduate of an approved nursing program and licensed in the practicing state.
