Job summary

Seaport Scripps Home Health is hiring a Registered Nurse (RN) to join our home health team in North Coastal San Diego County. The role involves planning, coordinating, and delivering nurse-led home health services with a focus on education and community health.

The ideal candidate holds an Active California RN license, has CPR certification, and at least 1 year of direct patient care experience (home health preferred). A valid driver’s license and reliable vehicle are required.

Qualifications

• Active California RN license (required).
• Minimum 1 year of direct patient care experience (home health preferred).
• Current CPR certification is required.
• Valid driver\'s license and insured, reliable vehicle.

Responsibilities

• Develop and initiate individualized care plans with patients and caregivers.
• Deliver physician-ordered nursing services and preventive/rehabilitative procedures.
• Educate patients and caregivers to promote health and independence.
• Identify and implement discharge planning needs.
• Coordinate equipment and services as needed.
• Supervise LPNs, LVNs, and HHAs.
