Job summary

Professional Case Management is hiring a Registered Nurse (RN) to provide in-home healthcare services in Ohio. You will develop one-on-one relationships with patients, helping improve health, quality of life, and peace of mind in the comfort of their homes.

Schedule is flexible with current availability on Tuesdays and Thursdays 8am-12pm, with potential for adjustment. Requires active RN license in Ohio, CPR certification, and TB test; new grads encouraged to apply and mentoring is offered.

• Excellent communication and interpersonal abilities to build trust with patients and families.
• Commitment to patient safety and adherence to infection control protocols.
• Ability to follow agency policies and maintain accurate documentation.

Responsibilities

• Provide in-home nursing care to patients in their own homes.
• Develop and maintain one-on-one relationships to support health, quality of life, and peace of mind.
• Follow care plans, document activities, and coordinate with supervisors as required.
