Job summary

Elite Home Health, a Medicare-certified home health agency serving communities across Florida, seeks a dedicated Registered Nurse (RN) to provide and coordinate skilled nursing care in patients' homes.

The RN will perform comprehensive assessments, develop individualized care plans, and coordinate services per physician orders, Medicare requirements, and agency policies. Prioritize patient safety, quality care, and regulatory compliance.

Qualifications

• Graduate of an accredited School of Nursing (ASN or BSN).
• Current Florida RN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Perform comprehensive and ongoing patient assessments.
• Complete SOC, recertification, and reassessment visits.
• Complete OASIS assessments accurately and on time.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing services per physician orders and plan of care.
• Monitor patient condition and identify changes in health or functional status.
• Communicate significant changes to physicians and care team.
• Coordinate care with physicians, therapists, LPNs, CNAs, and other team members.
• Provide direction and supervision to LPNs and CNAs where applicable.
• Complete accurate clinical documentation in the EMR.
• Ensure documentation supports medical necessity and regulatory requirements.
• Participate in care coordination, team meetings, and required training.

Position Summary

The RN provides skilled nursing care, completes comprehensive assessments, develops individualized care plans, and coordinates services in accordance with physician orders, Medicare requirements, state regulations, and agency policies.

• Develop, implement, and update individualized plans of care
• Provide skilled nursing services in accordance with physician orders and the plan of care

• Communicate significant changes and patient needs to physicians and the care team
• Coordinate care with physicians, therapists, LPNs, CNAs, and other interdisciplinary team members
• Provide direction and supervision to LPNs and CNAs as applicable
• Complete accurate and timely clinical documentation in the EMR
• Ensure documentation supports medical necessity and regulatory requirements
• Participating in care coordination, team meetings, and required training
