Job summary

Elite Home Health is seeking a dedicated Registered Nurse (RN) to provide and coordinate skilled nursing care for patients in their homes. This full-time RN role covers The Villages and surrounding areas, Florida, and requires an active Florida RN license with CPR certification.

Qualifications

• Graduate of an accredited School of Nursing (ASN or BSN).
• Current Florida RN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.

Responsibilities

• Perform comprehensive and ongoing patient assessments.
• Complete SOC, recertification, reassessment, and other required visits.
• Complete OASIS assessments accurately and within required timeframes.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing services in accordance with physician orders and the plan of care.
• Monitor patient condition and identify changes in health or functional status.
• Communicate significant changes and patient needs to physicians and the care team.
• Coordinate care with physicians, therapists, LPNs, CNAs, and other interdisciplinary team members.
• Provide direction and supervision to LPNs and CNAs as applicable.
• Complete accurate and timely clinical documentation in the EMR.
• Ensure documentation supports medical necessity and regulatory requirements.
• Participating in care coordination, team meetings, and required training.

Position Summary

Qualifications

• Valid driver's license, reliable transportation, and auto insurance
