Job summary

Transition Home Healthcare is expanding its dedicated team in the Houston, TX area. We seek a skilled Registered Nurse (RN) for home health services on a PRN basis to support multiple service areas.

Responsibilities include leading the nursing care team, coordinating with physicians and therapists, and developing patient care plans. QUALIFICATIONS include an active Texas RN license, 1+ year home health experience, and strong communication skills.

Qualifications

• Valid RN license in Texas or compact
• Home health experience (at least 1 year)
• Knowledge of home health regulations and general healthcare compliance
• Experience with Home Care Home Base (HCHB) EMR (preferred but not required)
• Excellent communication and people skills are essential
• Ability to work independently and as part of a team

Registered Nurse (RN) Home Health

Shifts Available: PRN

Service Areas with Openings: Northeast, Southwest, Southeast, Inner Loop
