Job summary

Transition Home Healthcare, a premier home health agency serving Houston, TX, seeks a dedicated Registered Nurse to coordinate patient care in home settings and lead the nursing team.

Shifts available: PRN. Service areas include Northeast, Southwest, Southeast, Inner Loop, with emphasis on compassionate, comprehensive patient care and accurate documentation.

Qualifications

• Active Texas RN license or compact license.
• At least 1 year of home health experience.
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR is preferred but not required.

Responsibilities

• Oversee nursing care team.
• Communicate with physicians, therapists, and other healthcare professionals to ensure cohesive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and families on managing health conditions, medications, and lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

Registered Nurse (RN) Home Health

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

Qualifications

• Valid RN license in the State of Texas or compact license.
• Home health experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
