Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Registered Nurse (RN) for home health visits on a PRN basis. The role requires coordinating nursing care, collaborating with physicians, therapists, and other healthcare professionals to deliver cohesive patient care across the Northeast, Southwest, Southeast, and Inner Loop service areas.

Qualifications

• Valid RN license in TX or compact license.
• Home health experience (1 year minimum).
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR (preferred).
• Excellent communication and people skills; ability to work independently and in a team.

Registered Nurse (RN) Home Health

Shifts Available: PRN

Service Areas with Openings: Northeast, Southwest, Southeast, Inner Loop

• Oversee nursing care team.
