Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Registered Nurse (RN) for full-time home health assignments in the Southwest and Inner Loop areas. You will lead the nursing care team and collaborate with physicians, therapists, and other clinicians to deliver comprehensive, patient-centered care.

The role requires an active Texas RN license, at least 1 year of home health experience, and strong communication skills.

Qualifications

• Valid RN license in Texas or compact license.
• Home health experience (1 year).
• Knowledge of home health regulations and compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
