Job summary

AccentCare, Inc. is seeking an RN Field Nurse for PRN Home Health in Fort Worth. The role combines direct and indirect patient care under the Patient Care Plan, serving pediatric to geriatric patients in home health settings.

Requirements include current Texas RN licensure, at least one year of nursing experience, and CPR certification. You will assess patients, develop and adjust care plans, train CHHAs and CNAs, and coordinate with physicians.

Qualifications

• Registered Nurse with current licensure to practice nursing in the state.
• Registered Nurse with minimum (1) year of experience.
• Possesses and maintains current CPR certification.

Responsibilities

• Assessment of the status of nursing needs of assigned patients, implements the individualized patient care plan, evaluates effectiveness.
• Promotes and maintains optimal client care; diagnoses and treats health problems as appropriate.
• Makes initial nursing evaluation visit and re-evaluates nursing needs as required.
• Initiates and revises the care plan, provides services requiring nursing skills.
• Assigns, supervises and teaches other clinical personnel, including para-professional orders.
• Informs physician of changes in the client's condition and counsels family and patient.

What You Need to Know

An RN Field Nurse is responsible for maintaining the delivery of quality patient care by assuring that the Patient Care Plan is followed. The position combines the activities of direct and indirect patient care and provides direct care to pediatric, adolescent, adult, and geriatric patients. The Staff Nurse directs CHHAs and CNAs in patient care tasks, performs patient assessments, develops patient care plans, evaluates the plan effectiveness, modifies it as appropriate and documents all patient care provided.

• Responsible for promoting and maintaining optimal client care. Diagnosing and providing treatment for human
• responses to actual or potential health problems.

• Re-evaluates the client's nursing needs as required.
• Initiates the plan of care and necessary revisions.
• Provides those services requiring substantial special skills within nurses scope of Practice Act.
• Assigns, supervises and teaches other clinical personnel, including written para-professional orders and
• supervision.
• Initiates appropriate preventative and rehabilitative nursing procedures.
• Informs the physician and other personnel of changes in the client's condition and needs.
• Counsels and instructs the client and family in meeting nursing and related needs.
