Job description

The RN is responsible for administering the Care Plan and managing the patient/client’s health and wellness as indicated by the Care Plan. Ensures compliance with nursing/clinical standards, federal, state, and local regulations, and company policies. The role involves treating patients when appropriate and as needed for Skilled Nursing Services. The RN will be assigned a primary office location with flexibility to assist and travel to other locations as needed.

Essential Duties and Responsibilities:

  • Follow the Care Plan in collaboration with the Director of Nursing, patient/client/caregivers, physicians, and care team members.
  • Conduct on-site Orientation, quarterly Supervision Visits, Skill Checks, and Annual Performance evaluations of home care aides, complying with regulations and policies.
  • Provide ongoing instructions to home health aides during field visits, including reinforcement of Standard Precautions, Client Respect, HIPAA, and Safety protocols.
  • Review Personal Care Plans from Aging Service Access Points (ASAP), and orient aides to follow these plans as authorized by the ASAP RN.
  • Document services rendered and any changes in client conditions or family and home situations.
  • Communicate with ASAP Case Managers or clients’ legal representatives/families regarding client status as necessary.
  • Perform Mantoux tests as needed for compliance.
  • Discuss medication changes with physicians; do not take medication orders over the phone. Redirect calls to the patient’s pharmacist/pharmacy.
  • Provide PCH/HHA/SHA training based on the approved training program.
  • Perform other duties as assigned.

Knowledge, Skills, and Abilities:

  • Excellent problem-solving and organizational skills; requires minimal supervision.
  • Competence in adhering to federal and state regulations.
  • Effective communication skills with staff, physicians, referral sources, patients, and families.
  • Computer literacy skills.

Required Qualifications:

  • Graduate of an approved nursing school; currently licensed as a Registered Nurse in the practicing state.
  • One or more years of experience in clinical home health services or equivalent preferred.
  • Valid driver’s license and reliable transportation with current insurance.
  • Current CPR certification preferred.

Work Environment:

  • Involves manual skills, motor coordination, and finger dexterity for treatments and injections. Occupational risks include needle puncture and back injury. Exposure to infectious hazards, including bloodborne pathogens.
  • Travel requirement of up to 30%, depending on business needs.
