Job summary

Avera in Sioux Falls, SD is seeking a Registered Nurse for dialysis with a focus on home therapy education and patient monitoring. You will assess patients, educate families, and coordinate care with the medical team to ensure safe treatment at home. Strong documentation and regulatory adherence are required.

This role includes on-call involvement, a Monday–Friday schedule with occasional nights and weekends, and a sign-on and student loan repayment options depending on eligibility.

Qualifications

  • Essentially requires active RN license in practice state.
  • Nursing experience of 1–3 years is expected.
  • Ability to educate patients and families about dialysis care.

Responsibilities

  • Assesses patient pre-therapy to determine eligibility.
  • Develops and delivers education on dialysis basics to patients.
  • Monitors patients monthly and guides families to deliver home therapy.
  • Facilitates access to resources for home therapy patients.
  • Coordinates care with Medical Director, families and staff.
  • Maintains accurate patient documentation and uses computer systems.
  • Adheres to CMS/ESRD regulatory requirements and safety standards.
  • Manages patient supply re-orders and inventory as needed.

Pay Range

The pay range for this position is listed below. Actual pay rate dependent upon experience. $32.50 - $48.50

Position Highlights

May be eligible for $10,000 sign on bonus! Monday-Friday 8am-5pm with occasional night, weekend, and holiday. On Call involvement.

A Brief Overview

Provides major portions of self-care and home dialysis training and monitoring for dialysis patients and/or their caregivers.

What You Will Do
  • Assesses patient pre-therapy to determine if patient meets selection criteria.
  • Identifies patient/family learning needs; Develops, implements and evaluates an educational plan for patients; Provides patient education related to but not limited to the dialysis process and procedures related the that process, including care and management of dialysis accesses.
  • Monitors and evaluates patients, on a minimum of a monthly visit, to problem solve/mentor patient and families to safely deliver home therapy.
  • Problem-solves and facilitates appropriate resources for patients on home therapy.
  • Coordinates care of patients in collaboration with Medical Director/Nephrologist, patients, families, other nursing staff and ancillary departments.
  • Maintains accurate documentation and computer skills.
  • Complies with established departmental policies and procedures, scope of care, performance improvement activities, safety, environmental and infection control standards.
  • Documents in the patient record according to departmental policy/procedure.
  • Promotes and maintains documentation and an environment that is in compliance with state, federal and regulatory agencies, including CMS/ESRD-Renal Network, and Quality Assurance and Process Improvement efforts.
  • Facilitates patient supply re-order.

Required Education, License/Certification, Or Work Experience
  • Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire
  • 1-3 years nursing

  • Certified Hemodialysis Nurse (CHN) - Board of Nephrology Examiners Nursing and Technology (BONENT) Upon Hire or
  • Certified Peritoneal Dialysis Nurse (CPDN) - Board of Nephrology Examiners Nursing and Technology (BONENT) Upon Hire or
  • Certified Dialysis Nurse (CDN) - Nephrology Nursing Certification Commission (NNCC) Upon Hire or
  • Specialty Certification - The American Society of Histocompatibility & Immunogenetics (ASHI) Upon Hire
  • Dialysis experience or critical care experience
