Job summary

Attend Home Care is seeking a compassionate Registered Nurse (RN) to provide high-quality care in patients' homes. The role offers part-time/per-diem scheduling, with competitive compensation and flexible shifts in Cleveland and surrounding areas.

The RN will perform SOC visits, ongoing nursing care, patient education, and care coordination with physicians. This position supports growth, team collaboration, and a patient-centered approach to reduce hospitalizations.

Qualifications

• Active Registered Nurse (RN) license in the state of practice.
• Experience in home health, skilled nursing, or related healthcare setting preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct Start of Care (SOC) visits and develop physician-directed care plans.
• Perform skilled nursing visits and reassessments to monitor patient condition.
• Provide hands-on nursing care including wound care, meds, injections, and chronic disease management.
• Educate patients and caregivers on diagnoses, medications, and preventive care.
• Collaborate with physicians and interdisciplinary team for high-quality care.
• Maintain accurate clinical documentation per state regulations and agency standards.
• Supervise and delegate care to LPNs and Home Health Aides as needed.

Employment Type: Part-Time/ Per-Diem (Flexible Scheduling Available)
Compensation: Competitive Pay, PRN: $125 SOC Visit / $75 Reassessment Visit + Mileage

Locations: Cleveland + Surrounding Areas

What You Will Do

• Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
• Provide hands‑on skilled nursing care , including wound care, medication management, injections, and chronic disease management
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
• Collaborate with physicians and interdisciplinary team members to ensure coordinated, high‑quality care
• Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
• Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care

What You Will Have

• Active Registered Nurse (RN) license in the state of practice
