Attend Home Care is seeking a compassionate Registered Nurse (RN) for home health in the Cleveland area. Part-Time/ Per-Diem roles with flexible scheduling and competitive pay, including PRN rates per SOC visit and reassessment visit plus mileage.

The role focuses on comprehensive SOCs, ongoing nursing visits, wound care, meds management, and patient education to reduce hospitalizations. The RN will supervise LPNs and HHAs, ensure accurate documentation, and collaborate with physicians to

Qualificações

• Active RN license in the state of practice.
• Experience in home health or skilled nursing preferred.
• Strong clinical assessment and critical thinking skills.
• Knowledge of home health regulations and documentation standards.

Responsabilidades

• Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans.
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed.
• Provide hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management.
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations.
• Collaborate with physicians and interdisciplinary team to ensure coordinated, high-quality care.
• Maintain accurate clinical documentation in compliance with state regulations and agency standards.
• Supervise and delegate care to LPNs and Home Health Aides to ensure adherence to the plan of care.

Employment Type: Part-Time/ Per-Diem (Flexible Scheduling Available)
Compensation: Competitive Pay, PRN: $125 SOC Visit / $75 Reassessment Visit + Mileage

Locations: Cleveland + Surrounding Areas

What You Will Do

What You Will Have

• Active Registered Nurse (RN) license in the state of practice
• Experience in home health, skilled nursing, or a related healthcare setting preferred
