Job Specific Responsibilities and Tasks:

The responsibilities outlined below are contingent upon departmental needs:

  • Patient Care Coordination: Collaborate with a multidisciplinary team of healthcare professionals to optimize patient outcomes in clinical effectiveness, quality of care, and cost efficiency.
  • Standards Adherence: Uphold compliance with established standards of care and practice as per the guidelines set forth by the MTF.
  • Clinical Assessments: Conduct comprehensive physical examinations and gather detailed health histories.
  • Health Promotion and Education: Deliver health promotion initiatives, counseling, and education tailored to individual patient needs.
  • Administrative Interventions: Administer medications, perform wound care, and implement a variety of personalized interventions as required.
  • Supervision and Direction: Oversee and direct care provided by other healthcare personnel, ensuring assignments align with the scope of practice and competencies of each team member.
  • Emergency Response: Identify and respond to adverse clinical signs and symptoms effectively in emergency scenarios.
  • Communication and Collaboration: Engage in productive communication with a diverse array of stakeholders, informing healthcare teams of care plans, educating patients and families, and contributing to organizational goals.
  • Referral Management: Organize referral appointments and coordinate specialty care as necessary.
  • Productivity Standards: Maintain a productivity level consistent with peers in similar roles.
  • Quality Improvement Participation: Actively engage in peer review and performance improvement initiatives.
  • Infection Control Compliance: Employ aseptic techniques and adhere to infection control protocols, including proper management of infectious waste and maintaining universal precautions to mitigate infection risks.
  • Safety Protocol Awareness: Operate with an acute awareness of safety procedures to ensure patient and staff protection.
  • Emergency Protocol Execution: Demonstrate proficiency in executing established protocols during emergency situations, maintaining composure, informing relevant personnel, and thoroughly documenting all events. Proactively identify potential emergencies and carry out appropriate preventative measures while notifying supervisors or designated authorities of unresolved issues.
  • Legal Compliance Awareness: Navigate patient care situations with an understanding of relevant legal issues, aiming to manage risks effectively.
Minimum Qualifications:
  • Education: Master’s degree required in Nursing, Health Promotion, Education, Community/Public Health, Wellness, or a related Health Science discipline from a recognized institution.
  • Experience: A minimum of two years (within the last four years) of full-time practice as a health educator is essential, focusing on Health Promotion and Wellness campaigns that encompass areas such as tobacco/nicotine cessation, diabetes prevention and management, weight management, stress management, sleep psychoeducation, nutrition, exercise, heart health, and disease management/prevention. For leadership roles, a minimum of four years of relevant experience is required.
  • Licensure: Valid, unrestricted RN license from any U.S. state.
  • Life Support Certifications: Current AHA or ARC Basic Life Support (BLS) certification is mandatory.
  • Security Clearance: Must demonstrate the capacity to pass a government background check/security clearance.
