Summary

We are hiring a FT RN for Home Visits/Admissions, Monday-Friday!

The Registered Nurse in Home Health provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The RN coordinates the total plan of care with other healthcare professionals involved in care and helps to maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources. All services are provided within a Point of Care setting.

Responsibilities

The RN provides and directs nursing care to patients in their homes, as prescribed by the physician and in accordance with laws, regulations, and agency policies.

• Coordinates the total plan of care with other healthcare professionals and maintains continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.
• Conducts initial and/or comprehensive nursing evaluations, determines primary focus of care, develops the plan of care following state-specific guidelines, and submits accurate, complete, and timely documentation in collaboration with physician orders.
• Performs comprehensive assessments at designated times and develops appropriate care plans.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by the payer source.

Education and Experience

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
