Job summary

Signature HealthCARE in Tennessee is actively hiring RNs for Westmoreland Care & Rehabilitation Center. This role focuses on direct nursing care, ADLs support, medication administration, and care-plan collaboration with an interdisciplinary team.

Ideal candidates hold an active RN license in the state and may have supervisory experience. Training includes EHR use, regulatory compliance, and opportunities for professional growth within a compassionate, purpose-driven culture.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the physician and responsible parties of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing or designee.
