Job summary

Signature HealthCARE is seeking an RN at Heritage Hall Rehab & Wellness Center in Kentucky to provide direct nursing care, supervise staff, and oversee care plans. The role emphasizes resident safety, documentation, and collaboration with an interdisciplinary team.

Candidates should have an active RN license, 1+ year of nursing experience (long-term care preferred), and familiarity with Federal and State LTC regulations. Strong communication and EHR skills are required for success in this role.

Qualifications

• Current, active RN license in the state of practice.
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Knowledge of Federal and State long-term care regulations.
• Strong clinical observation, medication administration, and documentation skills.
• Effective verbal and written communication skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
