Signature HealthCARE of Norfolk is seeking an experienced RN to provide direct nursing care in a 169-bed facility offering short-term rehabilitation and long-term care. The role includes supervising care staff, coordinating patient care plans, and ensuring high-quality resident outcomes.

We offer a $10,000 sign-on bonus for the 3p-11p shift, plus a $2 shift differential and additional support with continuing education and benefits.

Qualificações

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsabilidades

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs while maintaining comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents' status and report changes to physicians.
• Develop and review individualized care plans with the care team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities and regulatory surveys.

plus $2 shift differential and 10k sign on bonus for 3p-11 ft shift! How you Will make a Difference:

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.

• Proficiency with Microsoft Office applications.
