Job summary

Signature HealthCARE of Norfolk is seeking an LPN/LVN to provide direct nursing care to residents, including medication administration and wound care, with a focus on resident-centered care and quality outcomes.

Responsibilities include supervising nursing assistants, documenting care, coordinating admissions and transfers, and participating in QA activities. Virginia LPN/LVN license required; CPR/BLS preferred; 1+ year LTC experience preferred.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (AHA or Red Cross).
• Minimum one year of clinical nursing experience; LTC experience preferred.
• Experience in supervisory/charge nurse role preferred.
• Knowledge of Federal/State LTC regulations.

Responsibilities

• Provide direct nursing care including medication administration and wound care.
• Assist residents with ADLs while maintaining dignity and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents and report changes to physician promptly.
• Develop and review care plans with the interdisciplinary team.
• Complete clinical documentation per policy and procedures.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.

Additional Details:

$7500 sign on bonus for day and night shift

$10,000 for evening shift

How you Will make a Difference:

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference:

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
