The Pennant Group, Inc. is seeking a Registered Nurse to plan, organize and coordinate home health services in Colorado Springs and surrounding areas. You will perform skilled visits, coordinate care with interdisciplinary teams, and help manage patient plans within homes and communities.

Requirements include an active Colorado RN license, CPR certification, and 2+ years in home health or public health. A driver’s license and reliable transportation are essential for patient visits.

Responsabilidades

• Performs skilled nursing visits and coordinates client care.
• Acts as point of contact for all disciplines involved in patient care.
• Typically 3–5 visits daily; monitors frequency of visits per episode.
• Consults with physician and office to provide patient care details.
• Performs accurate OASIS collection and maintains up-to-date medication profiles.
• Attends weekly case conferences and monthly case manager meetings.

The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.DUTIES & RESPONSIBILITIES:Patient CarePerforms skilled nursing visits and completes coordination of client care,Point of contact for all disciplines involved with providing care to patients3 to 5 visits daily approximatelyOversees the frequency of visits for the episode.Consults as needed with the physician and the office giving details about patient care.Performs accurate OASIS collection, ensuring the medication profile remains current.Ensuring lab values have been reported to the physician timely,Attends weekly case conference and monthly case manager meetingsJOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)Must be licensed in the state of Colorado.Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.Maintains a current CPR certification.The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.Self-directed and able to work with minimal supervision.Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.Shows ability to organize and prioritize workload independently.Management experience not required. Responsible for supervising home health aides.Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
