Job summary

Benefit Home Health Care in Colorado Springs is seeking a Registered Nurse for home health visits. You will plan, coordinate and deliver nursing care in patients' homes, following community health goals and coordinating with multi-disciplinary teams.

The role requires Colorado RN licensure, CPR certification, and the ability to drive. Expect 3–5 visits daily with supervision of home health aides as needed and strong emphasis on patient education and care coordination.

Qualifications

• Must be licensed in Colorado.
• Minimum two years of experience; at least one in public health or home care nursing preferred.
• CPR certification required.
• Must have and maintain a valid driver’s license and reliable transportation.
• Self-directed with strong communication and organizational skills.
• Supervising home health aides is part of the role.

**SIGN-ON BONUS - $5,000.00**

RN pay rate per visit ranges $65.00 to $160.00

JOB SUMMARY

The registered nurse plans, organizes and directs/coordinates home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

DUTIES & RESPONSIBILITIES:

Patient Care

• Performs skilled nursing visits and completes coordination of client care,
• Point of contact for all disciplines involved with providing care to patients
• 3 to 5 visits daily approximately
• Oversees the frequency of visits for the episode.
• Consults as needed with the physician and the office giving details about patient care.
• Performs accurate OASIS collection, ensuring the medication profile remains current.
• Ensuring lab values have been reported to the physician timely,
• Attends weekly case conference and monthly case manager meetings

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

Must be licensed in the state of Colorado.

Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.

Maintains a current CPR certification.

The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.

Self-directed and able to work with minimal supervision.

Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.

Shows ability to organize and prioritize workload independently.

Management experience not required. Responsible for supervising home health aides.

Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
