Qualifications

  • Graduate of an accredited nursing program; BSN preferred.
  • Current RN licensure in the state of Michigan.
  • Minimum of one year professional nursing experience; home healthcare experience preferred.
  • Strong computer and technology skills.
  • Valid driver’s license and reliable transportation.

Position Purpose:
Join Trinity Health At Home as a Registered Nurse (RN) Float, where you’ll provide compassionate, high-quality care to patients in their homes across multiple counties in Southeastern Michigan. This PRN position offers flexibility and variety, allowing you to support patients with diverse needs while collaborating with interdisciplinary teams to deliver top-tier outcomes and satisfaction.

What You Will Do:

  • Conduct initial patient assessments and develop individualized care plans in collaboration with the Population Health Case Manager.

  • Deliver therapeutic interventions and manage ongoing care to promote independence and optimal health outcomes.

  • Supervise and support Home Health Aides and LPNs as needed.

  • Communicate effectively with physicians, case managers, and families to ensure continuity of care.

  • Utilize motivational interviewing and health coaching techniques to engage patients in their care.

  • Maintain accurate and timely documentation in accordance with agency policies and procedures.

  • Adjust visit schedules to optimize care delivery and operational efficiency.

Minimum Qualifications:

  • Graduate of an accredited nursing program; BSN preferred.

  • Current RN licensure in the state of Michigan.

  • Minimum of one (1) year professional nursing experience; home healthcare experience preferred.

  • Strong computer and technology skills.

  • Valid driver’s license and reliable transportation.
