Concierge Home Care is seeking a Home Health Registered Nurse (RN) in Jacksonville, FL for a full-time, flexible schedule. The role offers autonomy with 1:1 patient care in the home and an earning potential of $85K+ annually.

Coverage covers Jacksonville/ Duval County with a pay-per-visit model and potential quarterly bonuses. We support AI-assisted charting, a WellSky/Kinnser EMR, and ongoing clinical training.

Qualifications

• Active, unencumbered Florida RN license.
• Valid driver’s license and reliable transportation with auto insurance.
• Home health experience preferred, but not required.

Responsibilities

• Perform patient assessments and develop individualized care plans.
• Provide skilled nursing care and patient education in the home.
• Coordinate care with physicians and interdisciplinary teams.
• Monitor patient progress and adjust care plans as needed.
