What You Need to Know

RN Field Supervisor Responsibilities:

• Interpret and provide patient information documents such as admission packets, care plans, advance directives, Bill of Rights, consents, and patient education materials.
• Develop individualized care plans based on nursing assessments.
• Review reports from all involved disciplines, re-evaluate needs, and supervise care plan implementation. Conduct on-site competency reviews.
• Document patient status and progress accurately for ongoing home health services. Ensure continuity of care through effective communication with clinical and scheduling staff.
• Perform home re-assessments, monitor patient needs, and discuss changes with physicians and families.
• Provide 24-hour on-call coverage on a rotating basis.

Qualifications

RN Field Supervisor Qualifications:

• Current State License as a Registered Nurse
• Desired: One year of satisfactory nursing experience in home care
• Strong organizational, writing, communication, and interpersonal skills
• Self-motivated, dependable, adaptable, and productive with minimal supervision
• Ability to work independently, meet performance standards, and adjust communication style as needed
