Job Description

Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Job Responsibilities
  1. Assess the patient's physiologic health status: Conducts patient interviews, explains policies and procedures, reviews patient charts, and answers questions correctly and courteously. Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health status. Determines mobility, sensory deficits, prostheses use, and skin condition; adjusts plan of care accordingly. Assesses pain levels and management. Communicates and documents patient's physiologic health status and plan of care.
  2. Assess patient's psychosocial health status: Elicits perception of medical/nursing care, determines coping mechanisms, knowledge level, and comprehension ability. Identifies cultural/ethnic needs to reduce anxiety and ensure satisfaction. Documents psychological status and care plan, including support groups, counseling, and social services.
  3. Identify patient outcomes: Develops criteria for measuring outcomes, identifies actual or potential problems, determines teaching needs based on psychosocial and developmental assessments, and establishes individualized patient goals.
  4. Formulate the plan of care: Establishes priorities, coordinates resources, documents the plan, and collaborates with the healthcare team.
  5. Implement the plan of care: Ensures safety precautions, administers treatments, uses equipment correctly, provides emotional support, and responds to emergencies. Keeps accurate documentation and participates in discharge planning.
  6. Evaluate patient outcomes: Measures effectiveness of care, performs variance analysis, and designs systems for quality improvement.
  7. Collaborate with care team: Participates in planning, interprets treatment regimens, assists during procedures, and delegates appropriately.
  8. Perform skin and wound care: Performs decubitus care, applies dressings, compresses, irrigations, and other skin procedures.
  9. Administer medications safely: Identifies medications, prepares and administers them correctly, educates patients, and documents administration.
  10. Participate in education: Provides patient and family education, evaluates teaching effectiveness, and facilitates discharge planning.
  11. Perform related duties as required.
Physical Demands

Constant standing, walking, stooping, bending, stretching, lifting, pulling, pushing, and turning. Frequent lifting of objects from 10 lbs to 150 lbs, pushing objects up to 200 lbs. Occasional sitting. Assistance required for objects over 150 lbs.

Job Qualifications

Graduate from an accredited School of Nursing. Bachelor's Degree preferred. Must be enrolled in an accredited program within 24 months if hired after September 1, 2010, and obtain a BSN within five years. Current license to practice as a Registered Nurse in New York State. BCLS, ACLS preferred.
