Grace Healthcare Services in Atlantic & Cape May County is seeking a Hospice Registered Nurse (RN) to join our compassionate Interdisciplinary Team. You will assess and address patient and family needs, initiating interventions at admission and maintaining an adaptive plan of care.

The ideal candidate holds a NJ or multi-state RN license with at least 2 years of acute-care experience, and excels in communication, empathy, and independent work in a fast-paced hospice setting.

Qualifications

• Valid New Jersey or Multi-State RN license.
• Minimum of two years nursing experience in acute-care settings (medical, surgical, or oncology).
• Strong clinical practice with customer service focus and excellent communication; ability to work independently.

Responsibilities

• Deliver skilled nursing care per individualized care plans to terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans as needed.
• Educate and support patients' families and primary caregivers, providing guidance on care techniques and offering emotional support.
• Serve as a liaison between patients, families, and other healthcare providers, ensuring clear communication and advocating for patients' wishes and needs.

Role Overview

As a Registered Nurse (RN) at Grace Healthcare Services, you will be an integral member of our Interdisciplinary Team (IDT), responsible for assessing and addressing the comprehensive needs of patients and their families. Your role involves initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care throughout the patient's journey.
