Job summary

Avera Health is seeking an RN at Avera Diabetes Services-Sioux Falls to provide individual and group diabetes self-management education and to serve as a resource for nursing staff regarding diabetes care.

The role involves planning, conducting, and evaluating diabetes management processes, collaborating with clinics, and developing training for staff. CDCES certification is targeted within two years; on-call duties may apply.

Qualifications

• Active RN license in state of practice.
• Within 2 years of employment, CDCES certification is expected.
• Strong communication and patient education skills.
• Ability to work effectively in a multidisciplinary team.

Responsibilities

• Plan, conduct and evaluate diabetes disease management processes and systems.
• Serve as a clinical resource for providers and nursing staff in diabetes education.
• Support clinics in diabetes education access and program development.
• Identify opportunities for program development and participate in special projects.
• Develop and evaluate training programs for clinic staff in diabetic education.
• Collaborate with diabetes management resources across the system.
• Maintain expert knowledge in diabetes education and case management tactics.

Pay Range:

The pay range for this position is listed below. Actual pay rate dependent upon experience.

$27.75 - $41.25

A Brief Overview

Provides individual and group diabetes self-management education, conducts community and health professional education related to diabetes, and serves as a resource for the nursing staff relative to diabetes education and basic nursing care of the diabetes patient.

What you will do

• Plans, conducts and evaluates system diabetes disease management processes and systems.
• Serves as a clinical resource person for providers and nursing staff related to the care and education of diabetes patients.
• Supports clinics in disease management and access to diabetes education.
• Identifies opportunities for program development and participates in special projects as assigned.
• Develops, conducts, and evaluates training program for clinic staff to provide diabetic education.
• Collaborates with diabetes management and education resources across the system.
• Maintains expert knowledge in diabetes education and case management tactics.

Required Education, License/Certification, or Work Experience:

• Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire

Preferred Education, License/Certification, or Work Experience:

• 4-6 years of experience in disease management or case management
• Experience in disease management specific to diabetes mellitus
