Job summary

St. Croix Hospice. Rochester, MN seeks a Registered Nurse Case Manager to provide 1:1, whole-person home-based care. You will partner with an integrated care team to coordinate individualized care plans and support families through complex decisions with compassion and professionalism.

The role emphasizes autonomy in practice, on-call responsibilities, and collaboration with families to ensure comfort, dignity, and respect for patients at the end of life.

Qualifications

• RN licensed to practice in MN.
• 1+ years of acute care experience required; hospice/home health experience preferred.
• Perform comprehensive assessments to determine hospice needs.
• Develop personalized care plans with patient and family.
• Educate patients and caregivers on care techniques and medications.

Responsibilities

In the role of Registered Nurse Case Manager, you’ll be entrusted with providing 1:1, whole-person care to patients and families in their homes. QualificationsRegistered Nurse Case Manager Requirements & Responsibilities:

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Complete initial and ongoing comprehensive assessments to determine hospice needs
• Develop individualized care plans with the patient and family, updating as needs evolve
• Administer medications and treatments as prescribed
• Educate patients and caregivers on care techniques, medications, diet, and symptom management
• Rotate on-call duties and ensure seamless care transitions
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

The starting salary range for this role is $75,000-$101,000 annual base salary. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
