Position: Registered Nurse - RN Case Manager - Hospice - FT - Mon-Fri - Dayshift

Responsibilities:

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, and maintain primary control and professional management of each patient.
• Act as the primary liaison between physician, patient/family, and hospice team.
• Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload while fostering trusting and compassionate relationships.
• Empower patient’s caregivers through education on care for their loved ones.
• Manage and collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
• Evaluate patient and family needs and assist in the development and execution of the care plan.
• Maintain all assigned clinical records.
• Participate in quality assessment and performance improvement programs.

Requirements:

• Hospice heart: compassionate, kind, and empathetic.
• Comfortable providing care wherever the patient calls “home”.
• Demonstrate flexibility and composure during crises.
• RN license in the requested state or eligibility to obtain.
• 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
• Additional experience in hospital settings such as Med Surg, ER, critical care, etc., is a plus.
• Current driver’s license and automobile insurance.
• Current CPR certification.
