Position: Registered Nurse - RN Case Manager - Hospice - FT - Mon-Fri - Dayshift

- Hospice RN Case Manager experience is preferred

Responsibilities:
  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload while fostering a trusting and compassionate relationship.
  • Empower patient’s caregivers by educating them on care for their loved one.
  • Manage and collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
  • Evaluate patient and family needs and assist in developing and executing the plan of care.
  • Maintain all assigned patient clinical records.
  • Participate in quality assessment and performance improvement programs.
Qualifications:
  • Hospice heart: compassionate, kind, and empathetic.
  • Comfortable providing care wherever the patient calls “home”.
  • Demonstrate flexibility and composure in crisis support.
  • RN license in the state of employment (or eligibility).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital settings (e.g., Med Surg, ER, critical care) is a plus.
  • Current driver’s license and automobile insurance.
  • Current CPR certification.
